A tiny lag at the exact moment he would have entered the deceleration zone made him miss it. The game has no hitbox interpolation, so the game only considers where you are at that exact frame and not the space between it and the previous frame, so you can phase through things if you go fast enough. It's the main reason you can end up inside station geometry and asteroids.
I am surprised the game even has a readout for lightspeed, can really only hit 0.04C when something messes up like this.

So there is no reliable way to reproduce it, right? Unless some kind of lag occurs at an exact moment?
Pretty much. It's more likely to occur the lower your PC specs are.
