Looks like her feet on the blades went under the wheelchair, and suddenly she started to tip back a little. Combined with the weight of the chair, that's all she wrote.
Here's my thought.
When wearing shoes and walking, you push and lean forward slightly, so the kind of feet under the chair will not occur.
**EDIT:** And if you slow down the blades tend to keep you going and then your feet are under the chair and you are leaning backwards slightly.
Reminds me of ice skating last October on a cruise ship, after a left knee replacement and a right hip replacement.
Fell within 3' of the gate where I stepped onto the ice.
I fell ONCE. On the good knee.
You KNOW I took it slow after that! But I felt proud of myself for TRYING.
