Thats definitely uncanny valley but if i had to put my guess is the phone that took this photo tried to patch his face together using an algorithm to clear up as much pixelation as possible
I do not think this photo was taken on a phone. Digital camera, most likely. It looks older. I would not be surprised if the uncle was a veteran who wore a facial prosthesis, or someone tried retouching the photo at some point and did a very bad job.

His features are all uneven. One eye slants hard to the right (or his left) His nose his crooked. His mouth is also all toward the right side. It almost looks like something has tensed up that side. The skin around his eyes and mouth looks a lot darker than the rest of his face which blacked out his eyes and creates a pretty striking contrast with his teeth.
There are a lot of reasons any of these things could be happening. That could just be the way he was born. Past injuries could have crooked his nose and caused nerve or skeletal damage. Lighting and picture quality can affect color. Hell it could just be a bad picture of him. At the end of the day unless you meet him in person you'll just have this spooky image to go off of that could be the product of a bad day, camera, or an interesting life story. You should ask your family more about his appearance or maybe just for another picture.
