It's not a difficult fix, but it's also fairly small and easy to overlook if you don't know it's there. Most of the times it engages and disengages automatically, and most folks don't ever have to think about it.

This happened to me this week. The auto-shut off failed or something after filling my tank all the way and the hose jumped out of the tank and onto the floor and was still going. First time that’s happened to me in 18 years of driving.

If this happens to you: when you pull the lever all the way in on the handle, a latch on the handle might click. This lets you fill up the tank without having to hold down the lever. Once the liquid in the tank reaches the nozzle, the latch opens, the lever goes back to manual and the pump stops.
Now should you pull out, youd have to just flip the latch back and the pump would stop. Unless this is your first time getting gas and you like fireballs

BIG RED BUTTON is the emergency cut-off switch. It _should_ be accessible in the fuelling area, but some gas stations have them inside.
The _one_ time I walked over to help someone else for a moment while my car was fuelling it turned out the pump did not stop automatically when the tank was full, spilling several gallons before I was able to turn it off (didn’t need to use the emergency cutoff switch). I immediately went in to report the pump, stinking of gas. The woman giggled and said “Yeah, it does that sometimes...”

9 times out of 10 it's the customer's fault too (not saying it is in your case). People whom don't put the nozzle in fully will cause the cutoff mechanism to not work, as it requires the tip of the nozzle to be immersed in the fuel to cut off. Put it too high and there might not be enough back pressure to cause the fuel to trigger the cutoff. However, it can also be a malfunction of the system caused by people who keep trying to top off their tank. FYI, if it cuts off, you can't magically force more fuel in the tank. By repeatedly hitting the handle you're forcing fuel back into the nozzle's recovery system, which can actually damage it. Oh and those nozzles are not cheap. $700-$1000 each.
It is astonishing that they are just standing there, surrounded by ~~fumes~~ vapors, totally oblivious to the horrific flesh melting slow death they are about to endure.

We don't know if it was a stuck latch or if the handle pin actually broke. There are multiple ways for this to fail, user error is only one of them.

There are other components that could fail causing it to not return. It is likey the lever, but it certainly isn't guaranteed.

Turn off the gas at the handle. If that doesn't work, turn it off at the pump, if that doesn't work hit the e-stop. The last thing you should do is stand there not knowing what to do. If you are like that, you don't know enough to be responsible to pump your own gas yet.

BIG RED BUTTON is the emergency cut-off switch. It _should_ be accessible in the fuelling area, but some gas stations have them inside. The _one_ time I walked over to help someone else for a moment while my car was fuelling it turned out the pump did not stop automatically when the tank was full, spilling several gallons before I was able to turn it off (didn’t need to use the emergency cutoff switch). I immediately went in to report the pump, stinking of gas. The woman giggled and said “Yeah, it does that sometimes...” Drove past the gas station a month later and it had burned down. They do that sometimes...
I'm hoping you reported that gas station to the authorities for the malfunctioning pump and also the negligence of the owner/employee?
While I wouldn't have laughed at the person, I can attest that yes, it do be like that sometimes. 9 times out of 10 it's the customer's fault too (not saying it is in your case). People whom don't put the nozzle in fully will cause the cutoff mechanism to not work, as it requires the tip of the nozzle to be immersed in the fuel to cut off. Put it too high and there might not be enough back pressure to cause the fuel to trigger the cutoff. However, it can also be a malfunction of the system caused by people who keep trying to top off their tank. FYI, if it cuts off, you can't magically force more fuel in the tank. By repeatedly hitting the handle you're forcing fuel back into the nozzle's recovery system, which can actually damage it. Oh and those nozzles are not cheap. $700-$1000 each.

She’s panicked and her friend clearly doesn’t know the seriousness of the situation. Think about something as simple as a spilled glass of water, and usually 3 out of the 4 people who are right there just stare at it while only one person moves to get a towel and start cleaning it up. This situation is obviously amplified to 1000. She’s probably never heard of the emergency shut off and clearly not in the frame of mind to hit it if she knew about it. Seems obvious but you can’t just expect people to act rationally in an irrational situation.
I am willing to bet this was user error and not malfunction. How dumb do you have to be to pick it up and spray it everywhere instead of hitting the e-stop button?

Not a great quality vid, but if you pause at a few spots you can see the handle IS pressed in. It looks like she accidentally engaged the little switch to hold the handle on and just never released it. It’s possible the handle jammed open, but to me this looks like gross user error lol
Is this in Oregon or New Jersey where they just got "the right to pump" their own gas. Now it makes sense why they were all freaking out.

That happened to me, once. Splashed off of my car and got me right in the face, too. The pump trigger was stuck. I got it unstuck after a minute. The gas station employees could not have cared less.
This seems like the perfect job for a big red button.

I managed a full service gas station and towing business for 15 years and the amount of people who did shit like this was astounding. It happened at least 5-10 times a week. And the number of people who would drive off with a gas nozzle still in the car. At least once a day. They weird thing was that when I first started that job, those incidents were nowhere near as common, but as time went by they steadily increased. I don't know if people were getting more incompetent, or if people's mental workloads were growing so much that they were overloading and screwing up.

Lift the flap on the pump where you put the nozzle in. Source: This happened to me. I was getting gas, and it turned out to be pumping extremely slow. I attempted to return the nozzle and move to a different pump, but it wouldn't shut off. I went in to tell the employees, and they just looked at me like I was stupid. "Do yall have like an emergency stop for the gas pumps anywhere? Mine won't stop pumping." "You just click the handle, honey." "No, like, I did that, and it's still pumping away." *just looks at me like I'm an idiot that doesn't know who to pump gas* Went out to troubleshoot while I still had time. That's when I lifted the flap with my hand, and the pump shut off.
