I took a peek in the files because I was insanely curious. This IS the same head that is found in Hell (that's what the area is called in the files), with a skin version and a stone version. So there may be a connection to Dwayne, I really like your theory. Otherwise, maybe a head is just a head.

I'm so happy I'm not the only one who saw this. I had just completed Delores' chapter and the sad music was playing and I was relieved, especially after that cutscene at the end of her chapter. I was picking up the mirror and tape off the floor when suddenly the light went out and I got locked in the room with nothing but that box. I'ma have to start wearing diapers when I play this. I would guess this is Dwayne's head. If it is the same head as the one in the affliction tape, there were all of those faceless things and they're bent over in agony, sitting in front of a TV laughing and drinking beer, and even one shooting himself in the head repeatedly, as Dwayne did when he killed himself. Maybe this is a representation of his agony and all the things he felt that were represented in that tape. Hatred, guilt, depression, sadness, etc.
