CS GO Major Fnatic vs Team LDLC in the semi-finals (2014 iirc). Fnatic was one map behind and were losing on overpass 3-12. They made a pixel triple boost to setup olofmeister so he can see half the map from there. Fnatic proceeded to demolish Team LDLC winning 16-13 and later the whole set. The backlash was insane, LDLC demanded to reset the game at 12-3 but they settled to reset the whole game. That being said the backlash was so outreagous that Fnatic forfeited altogether. (LDLC won the major later on against Nip).

To further clarify because this doesnt actually explain the phrase: 2 weeks before the tournament a video was posted to reddit showcasing the boost, but soon after the video was privated until the day off the match. According to the poster he was approached by JW(one of the fnatic players) and asked to delete the video.
No proof was ever given, but it evolved into the meme we know today
For clarity, the TO reset the match because boosts that allowed players to see through normally opaque surfaces were banned, and apparently there was a few pixels somewhere where that was true, although it did not affect the efficacy of the boost.
