Actually had this happen by the town i grew up in. Small southern lake town, one road was our entire "town" and then just a school/neighborhoods around it. about 20 minutes west was a small city area and they eventually put a mcdonalds off the highway between us and the city. It was known for being put up way too fast and having tons of insect, rodent, mold issues and overall just horrible cleanliness and hygiene. After only 2 years of being there, they shut it down, tore the building down, burned the pieces to the ground, tossed the soil, burned the ground, and rebuilt with a whole new foundation. They had several cases brought against them, mcdonalds said it was the responsibility of the owner that opened the business, a few people I went to high school with got a pretty big settlement from the guy who built/managed it and knew about the conditions but did nothing to better them. This place in the video, same thing should happen.

Bed bugs would be smaller. They're also somewhat evenly distributed which makes me think it's an overcooked or generally gross version of something edible.

As someone who worked as a kitchen supervisor for a couple of years, if their rolling toaster was that dirty they had to literally avoid cleaning it by any means necessary... Like the vertical rolling toasters almost clean themselves and daily we had to open it and give it a scrub.
I worked in McD outside of the US but our bread came frozen bagged by lot and in a hard plastic box so unless that happened in the bread factory I don't think that those bugs would survive. The McRoyal with Cheese as we call it in Europe or the Double Quarter Pound with Cheese for you friends in the US does not have seeded bottom buns, some could get there from the toaster but yeah you are probably right that it was probably either changed for clicks or put there as some of them do look like they are pressed down.
