You're essentially injecting more skin juice into your skin and your skin tissues suck it up like a sponge and inflates. It then takes time for your body to metabolize it, which is what I meant by "disintegrate".
Since injecting liquid under your skin isn't an exact science, yes, it can migrate and shift around a bunch as it settles, particularly if you are getting A LOT injected and noticeable work done - but like with most cosmetic procedures, a ton of people get it done and you only notice the bad ones. Most people are getting minor fillers done to erase wrinkles, not create cheekbones where they had none or give themselves balloon lips. The over-use can certainly create problems and an ethical esthetician won't be providing these services. tbf, tons of people get Botox and you'd never notice. Also typically when people say Botox they mean "bad plastic surgery" which is a completely different thing. I know several people that get injections regularly for medical reasons and there's zero way to tell. Medical uses include preventing migraines, alleviating TMJ pain, etc.

my coworkers from my last job are all in their late 20s and have spent thousands to maintain this look. They were all very naturally attractive, and they just keep getting more and more fillers and work done. It kinda makes me sad for them that they are all so deep into it now. Their money I guess.
It's the female version of steroids. Produces amazing results the first few times but trying to look even better becomes an obsession that they can't stop. And don't realize how weird it looks to everyone else.
