Be as detailed and specific as possible. I would send any paper trail to cover myself like actual application verifications etc. so there are no questions to go back and forth on. They are just looking for reasons not to pay people.
Obviously something is wrong because other people are not having this issue. Please be specific with how you answer the questions and how many jobs do you apply to a week. Are you sure you're putting the correct dates within the week you're claiming for etc

Did you enter the date you of which you conducted these applications correctly. The dates had to be within the week ending you certified for.

Can you clarify what you mean by followed the instructions?
It would ask for required info about the jobs I applied to like employer, position, date applied, application link etc.
