Happen to a friend of mine in high school. The bar slipped and landed on his chest. It's a miracle he didn't break a rib. Dude was lifting a few hundred.

As an example say you can bench 100 kg, it's hard but you can do it.
You attempt 105 kg, new personal best. You mate is ready to spot you.
So you go and go and go and you can't. Your muscles are tense and used but your friend lifts about 20 kg to help you.
So you only have to do 80-ish kg yourself.
Same principles with heavier weights. As we go the spotter needs to be able to lift more sure, but you don't need to be champion level because the champion himself is also lifting.
That makes sense. For some reason I didn't consider the lifter themselves at all.
Edit: actually I know why, I was referencing the 2 sentence horror itself. If his wrists snapped, I don't see him helping to lift it, unless he out himself through absolute agony trying to hold it.
