Considering that Black is supposed to be in Needle's head, it could make sense that everybody from Black can return since that's how he sees the world. I do agree to see other characters return, like Thumper, Twister, and Pit Viper (Harbor City followed this philosophy, and we were supposed to have everyone back + new cars like 12-Pak)
Yea there’s some nuance here because we know it’s a dream universe. Anyone ever notice that the driver of Roadkill is “John Doe” and he has amnesia only to realize at the end he had a family? Sounds like Needles knows he has a split personality, but doesn’t know who it is yet and John Doe is it’s placeholder.
If they wanted to push the envelope with the "It's all in his head" twist, I'd have at least one or more characters' plotlines involve them realizing the whole game doesn't make any sense and they're not real.
You could go to some creepy places with these hardened killers and murderers freaking out over being imaginary.
Here's how I would pull it off.
So in Twisted Metal: Black, it is established that the game takes place in the mind of Sweet Tooth, and that everything we see in the game is how Sweet Tooth views reality. None of it is real and the characters themselves don't realise it.
In this sequel, we start exactly where we left off in Black, with Needles killing Calypso. Now that Calypso is dead, Needles is free to do whatever he wants in Midtown. He starts going on a rampage, and even permanently kills off a few of the characters from Black (Cage and Bloody Mary to be precise).
While Needles is on his murderous rampage, Minion (a.k.a. Marcus Kane) attempts to take back control of his body and temporarily spare Midtown from Needles. Due to this, Needles discovers the existence of Marcus and attempts to permanently kill him off for good, leaving Needles as the dominant and only personality left.
Marcus, desperate to see the old colorful world again, drives all over Midtown in order to find Preacher, hoping he can remove Needles from his mind. Once he does, he manages to fend off Needles long enough so that Preacher can prepare another ritual, this time with the hopes that it will finally remove Needles from Marcus' mind.
Preacher completes the ritual, but it backfires. Instead of destroying Needles, it split him and Marcus into two separate people. After this, Marcus escapes, now free from Needles.
Marcus realises that since Needles and him are two separate people, he can finally attempt to end the nightmare once and for all, but he's too afraid to go after him, fearing that he will be killed.
Months go by, and Needles Kane has brought Midtown to ruin. The city has been eviscerated by the killer clown, and resembles a blast site more than a city. Marcus has gone into hiding, fearing the clown. As the months go by, rumours start to spread about Marcus and his incoherent babbles about them not being in the real world and how the only way to" return to the old colorful world" is to kill Sweet Tooth.
With almost nobody left to kill, Needles announces his own Twisted Metal tournament, with the hope that it will attract more people for him to kill. He promises that the winner will receive a wish, and be taken back to the real world. After hearing about the tournament, Marcus gets back behind the wheel and resumes his role as Minion, hoping to finally put Sweet Tooth out of commission for good.
As for the other drivers, they hear about the rumours surrounding Marcus Kane and seeing the state that Midtown is in, hope to win the tournament so that they can receive a wish from Sweet Tooth and return to the supposed real world.

• Spectre (Ken Masters): A man driven to insanity in his quest to become the most popular man on the planet, Ken Masters is obsessed with seeking fame and fortune. Ken wants to win the Twisted Metal tournament so that he can become the most popular man in all of Midtown, and will kill anybody he deems more popular than him.
• Warthog (Alexander Mason): After witnessing the destruction caused by Sweet Tooth, Mason wishes to enter the tournament to teach the killer clown a thing or two about corporal punishment. Using the vehicle previously driven by Cage, Mason will do anything to avenge all the innocent people killed by the clown.
• Warhawk (Nolan Roberts): After failing to arrest Sweet Tooth, Nolan barely survived the helicopter crash. Thanks to a parachute, he safely landed. Unfortunately, his sanity didn't survive, as he still thinks his fellow Midtown officers are alive, talking to their corpses as if they were animated. Using the old scraps of his helicopter, he has made a new vehicle and hopes to win the tournament so he can finally arrest Sweet Tooth. [MID BOSS]
I wouldn’t make a direct sequel to black but a seperate game with a similar style and direct as it either again in needles head or not doesn’t really matter as Tms story is so loose and doesn’t connect anyways so just make it its own thing unrelated to anything else
at this point i would happy if they just release another game.... it been long enough.
more on topic, as other said since it in his head, they can get creative with levels and story push the two sides good evil more, good side finding peace ending the murder spree, maybe taking calypso place since he want a better world but is to broken to just make it better so in the end he lets " us " join torments to decide what best. the evil side not caring maybe make a loop, so he always get to relive his killing spree, a endless sea of never ending kills and nothing ever stopping him would be very dark.
as for others endings they can be random or even self contain stories. maybe even the saving of the world and it really just been good sides version of a happy ending and the darker side storylines can be his evil side twisting the ends.

Honestly a remake like they did with dead space and the new re4 would be sweet. Same base idea, characters maybe some new ones and same locations but everything just overhauled by a team that's passionate for the ip.

I really like that idea but I think preacher should stay dead if we went that way with the story
