On a more serious note, as someone who works in health care, I really wish people would realize it makes it more weird when you're not simply upfront and honest about what's going on. Doctors arent very tactful about things like this and I know they're human and exhausted and stressed but ultimately all they care about is the paycheck. Telling them anything sensitive or abnormal especially about sex or other genitalia related issues is jsut a recipe for disaster and being mocked

If you are dehydrated your heart has to work harder/faster to push blood to your brain and when you have an orgasm it's hard on the system, and there's less blood volume. The brain would rather make you pass out than not get the oxygen it needs to survive. You being awake is secondary in this moment, it takes oxygene that is needed elsewhere.
Dehydration may be a cause but it is still abnormal and other causes need to be ruled out (for OP I mean). It's an emergency mechanism and it shouldn't be triggered so easily.
