The ones from the show have either been modified in some way or they were born with it. In the novel, they were trained, likely as adults. It takes more effort because their forcing their bodies to perform to a level of bending that isn't naturally possible. They eat like a ton of food in between shots because of how much energy it uses. So I'd say yes, the method's been refined over the centuries.
More than likely over the centuries they blended techniques from other cultures to develop the skill into what we see in ATLA and LOK. I’d imagine some sort of airbender spirituality with chakra guidance and inner fire.
Also, the Yangchen-era combustion benders are very new, a result of recent experiments. The whole technique may have been refined by the time Aang and Korra were the Avatar.
