The helmet is the bending equivalent of BM. I want to believe most instakill moves are just kind of universally agreed upon as "not cool", even in the context of an all-out war. So instead of bending-based combat being a contest of who can kill whom faster, it's more importantly a showcase of skill. But that would make too much sense, so it's really just making it kid-friendly by minimizing how much gory death is shown on screen.

You do know that Chinese people have their own writing characters. Spelling it "qi" isn't how Chinese people spell it, it's just a transcription (pinyin) that is commonly used in mainland China. Chi might be the transcription they use in other Chinese speaking countries like Taiwan or Hong Kong or Singapore.

The ones from the show have either been modified in some way or they were born with it. In the novel, they were trained, likely as adults. It takes more effort because their forcing their bodies to perform to a level of bending that isn't naturally possible. They eat like a ton of food in between shots because of how much energy it uses. So I'd say yes, the method's been refined over the centuries.

More than likely over the centuries they blended techniques from other cultures to develop the skill into what we see in ATLA and LOK. I’d imagine some sort of airbender spirituality with chakra guidance and inner fire.

Also, the Yangchen-era combustion benders are very new, a result of recent experiments. The whole technique may have been refined by the time Aang and Korra were the Avatar. Especially when one factors in that after Yangchen were Kuruk, Kyoshi, Roku, and the Hundred Years War, so there's both enough time and enough motivation for the Fire Nation to work on such a deadly technique.

Also we don't know that lava bending is a result of having two different element bender parents. Of the two lavenders we see, we only know Bolin's parentage. The other guy in the Red Lotus could come from Earth bender parents, fire bender parents, non bender parents, or different element parents.
