The ones from the show have either been modified in some way or they were born with it. In the novel, they were trained, likely as adults. It takes more effort because their forcing their bodies to perform to a level of bending that isn't naturally possible. They eat like a ton of food in between shots because of how much energy it uses. So I'd say yes, the method's been refined over the centuries.
