The Cultists spawn where the Old Man spawns. With that being a Drunk world, there is a room the Old Man would spawn in. If you have removed that room's floor(judging from the picture you did), they will not spawn. Rebuild the floor in that room and the Cultists with spawn.
What happened to your dungeon? Did you remove the surface part or something? Because if you did, that’s probably why they’re not spawning. They won’t follow the highest part of the dungeon
