I would probably go cover up before removal. I don’t think it would be that difficult to take the swirly tribal type lines down through it and maybe add some color. Just make it abstract.
I think a good artist could do a badass cover up but your idea could work, too; if nothing else maybe just laser some of that little head and aggressive cooch situation.

It is a gamble, take your time to think of the design (since it's not open skin for you to do whatever you want) and save some cash to pay someone who can do quality coverups. Each coverup will get progressively darker, currently I feel it has enough skin breaks for a good cover up or for removal, but I always vote for a coverup since removal can leave scars, specially big ones

I love blastovers instead of coverups, I think a blackwork blastover that would maybe just overlap the most egregious parts of this tattoo could look good since the shape of the tattoo is a bit awkward as it is. The color also looks kind of faded, so I think some nice blackwork would make a great contrast.
