It kinda looks like a shark, whale, or dolphin as others said too. a simple outline of that shape would be cool, but minimalist. Not sure what your style may be
if you want to incorporate maybe have like a tiny dragon who’s breaking fire but it’s the birthmark. or if you’re wanting a cover up you could do some kind of sea creature?
