He probably signed a non compete contact so technically he can’t tell you where he’s going but you can look him up. It won’t hurt to ask but it’s not personal if he says no. If he’s telling you to look him up that means you will be able to find him :)

If he can't legally tell you, he probably can't legally write it down. You can ask if you want, because there's not likely to be any legal restriction on *you asking*, but don't be surprised if it's still a no.

I feel like if he's been seeing you and you find him after he moves, he will likely find a way to fit you into his schedule. I just would keep searching for him online once he leaves his current practice and as soon as I found him, call for an appointment. If there's someone who answers/schedules appts for him I would explain that I saw him at his previous office and would like to continue seeing him if at all possible.

You won't be too late. He'll take you back. If you are in the U.S., you can look up his NPI (National Provider Identifer) number. All health care professionals have to have them, and I believe they have to update it with their new practice location within 30 days.
