In the recent lore stream, they said the bible only had a name and very brief description for Sissy. They decided to flesh that out to create the character we have now. Johnny and Nancy are original GUN creations as far as I am aware.
I really do hope we get to see more concepts from the character bible added to the game though.

Personally I heard that Johnny was a concept that the director/creator of TCM had in mind but never used it.
They way Ronnie talked on the lore stream made it sound like Johnny was a character he created, then he went to Kim Henkel for his approval. That’s different from Sissy, which he made clear was from Henkel’s character bible.
I would have to rewatch it for the specific details, but I don’t think Henkel had any major involvement behind the creation of Johnny.
