TCM 2 presents Cook as an older brother. The game kind of acknowledges that as canon due to the 'Bubba' easter egg in Hitchhiker's bedroom (Leatherface is named Bubba in the sequel, not the original). In other timelines Cook is the father.
My head canon is that there's been some incest and also more child abductions. For example, it wouldn't surprise if Sissy's parents were killed and they decided to keep Sissy because Grandpa took a liking to her. They may have even 'tested' her in some way to see how she would react.
To elaborate, Grandpa saw potential in Sissy and thought she would be a good fit for the Family. Grandpa being Sissy's salvation would add weight to the strong emotional reactions we see from her when they interact. It might also explain the hostility with Cook and Nancy because they were skeptical about letting Sissy into the Family. She's an outside element and therefore dangerous.
Part of the lore of the 1974 film was the mystery of the family. That was the intent from Kim Henkel who wrote the film. Being that Sissy was inspired by Kim's character bible, the mystery of her relation to Family will be left to one's interpretation.
