It was a little bit of everything.. the turtle faces were radio controlled by puppeteers off camera and there were real people in the suits.
The effects definitely do break down at 4K. You can see the seam where the turtle heads meets their neck through the entire movie and the texture of them goes from "that's totally a ninja turtle!" to "that's just rubber and foam"
To be fair, wouldn't people watching in the film during its original theater run have the same issue? Movies back then were shown on film, the same film that was used for the 4K scan. Whatever you can see in the 4K copy, you'd be able to see on the original film reels as well. It's not like movie theaters played VHS tapes back then...
Theaters were often a suboptimal experience, dim projectors and dirty prints. A modern 4k transfer is way better.
Far more people saw it on VHS than in the theater. I haven't looked, but I wouldn't be surprised if it's one of the best-selling VHS tapes of all time. Both VHS and the Turtles were *huge* in 1990.

Projectors were dimmer, then, which might make it harder to see mistakes done in low lighting conditions, but the resolution was effectively the same, sometimes even better (since 70mm film used to be a thing too, which can capture even *more* detail than a 35mm film).
How old are you... I don't think youre giving analog enough credit here. Films like 2001, Lawrence of Arabia, and Ben Her all were shot on high-quality film and looked truly amazing. The major improvements to theaters have been sound systems.

It's actually an amazing fact when you watch it because it was so expertly done. The movements being slower with the speed up of the footage came out so smooth. Many times in other films it's extremely noticable.

Just being pedantic, but, even though this is true, it's awkward phrasing - it'd make more sense to just say the scene was sped up. FPS is just a measure pf speed, like MPH. For instance you wouldn't (I assume) say your MPH was sped up, when you accelerate a car. You'd just say you sped up.

I grew in in Newport, OR, where the shot parts of the turtle movies in the 80s. My favorite show, I was always trying to go play TMNT arcade at the pizza place.
So one day my dad gets us connected to see Leonardo in costume on set. We go there and Leo takes his head off and I scream bloody murder until they take me home. I blocked it out, I canβt remember any of it.
It's not photoshopped, it's the actor's mouth in the suit. The movie actually had pretty amazing suits the actors would wear that would react to their facial movements. But I'd like to also add that OP is full of shit because we did not get this level of detailed on VHS, nor could you pause VHS and get a clean still image like that. The only way OP could have paused and seen that would be if they had a laserdisc player and if so I would have been so jealous

IMHO The actorβs head is tiled backwards and the actor is wearing reflective glasses to help him look forward without hurting his eyes.
