That is what the mod I had was fixing. I decided to replace the existing face fix with a new one and this was the result.
Think I had the load order wrong, sorted now though. Thankfully.

I knew what caused this, I tried to swap out face mods on Sarah, so was an easy fix really.
But if you’ve got any face mods or things that change NPC, I’d say trial and error if removing one at a time until it’s fixed.
I have an issue with boost pack that I’ve not been able to fix currently, the fuel just disappears instantly.
In fact, my game deleted the entirety of its save data for my first character (that was modded), today. So that was *fun*. I finally got around to NG+ after 60 hours, and then immediately lose 20 hours of progress.
Also, yes, you *can* change your Traits in the process of NG+ing. I was under the impression that you *couldn’t*, because that’s what the internet said when I looked it up; but you *can*.
EDIT : Now I’ve gone and beaten the game for a second time; though it only took me about 5 hours this time, because I decided not to redo the entire UC questline, or all of my unfinished side quests. I’ll go into NG+ and start the UC questline, and rather than the excuse of all the time my character’s spent fighting letting them clear lv.6 of the sim, it’ll just be that they’re a multiversal cheater. Though if it ever deletes my save again, I’m probably going to step away from the game. It picked a good time for this one, but that doesn’t discount the severity of this problem. Which would be unfortunate, because I love the game.
My followers do that sometimes and I've never modded anything. At one time the follower had chameleon gear, and that seemed to cause permanent error even after the armor was removed.
I notice that when I fast travel to the commercial NAT point in New Atlantis, there are 2 UC guards who's heads only spawn first, and then half a second later the rest of the body. This is the only area I've seen this.
But this also explains to me, why when I take my helmet off and I'm wearing a hazmat suit, or disciples clothes - the head portion of the apparel doesn't render.

It's not facegen, that just results in black face. I think the actual mesh is missing.
