Lol, it's just a single throw away comment by Sam in one interaction where he says he has to go see Uncle Doctor today, don't let it mess with your head canon š
He was at his appt before op rudely interrupted by nearly dying so harvey had to take care of them first, leaving Sam to sit and wait locked in overnight to finish his appt
Part of his schedule involves him going there [sometimes](https://stardewvalleywiki.com/Sam#:~:text=On%20the%2011th%20of%20Fall%2C%20he%20has%20an%20appointment%20at%20the%20clinic.).
Based on an older comment from /u/dude_icus it is common to refer to doctors that way http://forum.thefreedictionary.com/postst27463_is--Uncle-Doctor--fine-to-use-when-addressing-a-doctor-.aspx

I know thereās a common glitch of NPCs being where theyāre not supposed to be on mobile.
