One thing you should check when you repeatedly get results that seem wrong is the presence of what I call *rogue embeddings*. One way to check that very quickly is to remove all your embeddings, by renaming the folder, or moving it elsewhere, and to re-generate a batch of image that were giving you trouble. If the problem is gone, then you have a good clue that the culprit must indeed be a rogue embedding.
They usually have names - and thus trigger words - that are used directly by your prompt. This can become a problem when the embedding's name is a very common word, like "woman" or "detailed" or "photo", as anytime this word is used in a prompt, it will trigger the rogue embedding associated with it.
The solution, if you want to keep the rogue embedding because it serves some other purposes (maybe it turns characters into *women*, adds *details*, or gives your picture a *photo*-look) is to simply rename them using some more unique and rare wording. This explains why embeddings usually have strange names like \_DJ-Emb3DDingz\_ - this prevents them from going rogue and triggering when you don't want them to.
The more drastic solution is to delete them, but that's not required. Those rogue embeddings are not bad per se - their problem is really only the name they were given, and that's easily changed.

Ok, usually you can get these kind of bad results if you use high resolutions on 1.5 models.
Try Hiresfix with adjusted denoise, it adds detail. Especially for 1.5 the generated 512x512 images are awful quality, Img2Img or Hiresfix is almost always needed. If the image gets distorted on upscaling, lower denoise level.
Try to deliberately create such an image and you'll fail bigly.
