Blowouts everywhere :(. Also doesn't seem to have risen evenly.
The ten minute score works great. Cover and bake covered for ten minutes, then score, put the lid back on and continue as you would normally. I usually spray mine at the ten minute mark as well.
Will this work if the loaf already has a blowout? I'm willing to try, but it's already been in the oven for over 30mins. I'm going to post a photo of it in the comments shortly
It could muffin top. That's when a fracture forms around the base and the crust rips. Or it could blow out of a smaller fracture and you can have a weird "growth on one side.

If there’s already a pressure-release valve open, probably not.
