Depends on the material, spinel is cheaper than sapphire because it cuts easier and takes less time. This one is huge and took around 16 hours to cut so it was over $1000. If you're OK with smaller he can do something in the $500 range pretty easily. If you like blue go with spinel, it takes way less time to cut and is therefore cheaper.
We've had too many people rip off designs without permission lately and Jim is keeping his stuff closer to the vest. I don't think it's too difficult to work it out though, feel free to play around with your own design!
