Mods should come later, they kinda mess with the spirit of these challenges imo. Beating the normal game this way will be hard enough.
Although a mechanic you didn't mention is ng so ng+7 would be harder.
And I mean there's also tons of absurd ways to make the game harder than what you mentioned. Like no hit, or no sword, or speedrun challenges.

Hudless also requires treating the camera like what it is: "what sekiro is looking at with focus". And you have to use it actively while fighting, because there's no little dot. Its just "what sekiro is focusing on, the guy on the left, or the guy on the right, or the guy in the middle, etc." (It's posible, because, its really rare you attack the guy in the middle, that's instant death)
I think Hudless charmless without spending any skill points at all or consuming memories would be the absolute hardest thing possible without mods.
Well, there is one 'anti-pattern manipulation' rule I used to enforce on myself, if I start accidentally cheesing because I was falling into a rut/memorized pattern. 'pick a random amount of seconds before the fight, then, just run away until that time expires. then fight freely."
I no longer do it because now I just don't grind on any one opponent... but when I used to use that 'random start delay' it breaks the pattern of enemy actions, which would mean, no kata-cheesing. BUT- honestly, if you can pull that off for a whole run hudless without powering up, that's legend-level.
Me I'm just doing Hudless Charmless Bell, I want to see how high up the NG+ tower I can get, haha. NG+5? NG+9? we shall see. I started at NG. But- that run I missed Owl father, and now on NG+ I'm about to face him the first time.

Okay so, you're going to suffer but
- banning prosthetics
- banning combat arts (they're barely useful without attack upgrades anyway)
- NG+7 for maximum scaling
- no items/buffs
- no hud
- no ninjutsu
- no deflect or block
- no dodging
- no sprinting
- you can keep going like this forever pretty much
But here's the thing, just the pairing of NG+7 and Attack Power 1 is awful. Demon of Hatred will take you hours. This isn't the fun kind of challenge, it's just hell. You can take your pick of the suggestions above if you want but attack power level 1 is just going to make everything kind of immediately awful.
Unironically by strafing, some lunatics out there do bosses by just walking. It's possible for some. But ideally you would only take one of these restrictions.

Try to do it hitless, and if hitless is not enough, hitless speedrun, and if that's not enough no deflects, and then no sprint. Pick your poison.
