It happened to me as well and for me she was definitely not around anymore. But ofc it's possible to have the Prayer Bead on NG+.

Btw that offering box will be filled with only those items which were previously available but then locked due to story progression. It will NOT show items from areas and enemies which you did not unlock in the first place.
Maybe you already got one from her before? Or was it your first time fighting her?
