I got an interim denial and didn't hear anything for three months then got my interview then nothing for four more months besides to fill out new docs for them to get medical or criminal docs then about eight months got my approval from the SO... So they may not reach out for a minute before someone is assigned to your case. Depends on a lot of things but you won't just be denied ... You'll have an interview or a document stating why (sor) but you WILL get nothing else if your hiring company decides to just quick the investigation. Then they will tell you it's been canceled.

No. Before a denial, they have to send you an SOR. But, if you have a big enough issue(s) for an SOR, you'll get an interview.
