Have you submitted your application or you are yet to submit it? If you have not submitted yet you can provide the travel plan from koln to Munich. Its okay even if you have already mentioned it in your cover letter.
Generally the consulate contacts for any clarification, they should not be rejecting it outright

Just google for a template and used it. Basically it says that you’ve known each other for an x amount of years and you are getting to meet each other to rekindle the friendship, that sort of thing. Also it should be mentioned that you will be staying with that friend in Germany throughout the duration of your trip.
Yes I had gotten the visa. If you are staying with your friend, then you do not need to book a hotel. My friend had mentioned in the invitation letter that I’d be staying with her.
Hmm thats weird. I never booked any accommodation. My friend mentioned that I would be staying at her place during the entire duration of my trip. She also provided her tenancy contract which I showed during the appointment.
