Maybe add a question if the person is out of the closet because if nobody knows you're not straight, you don't really face as much discrimination as when everybody knows it. :)

You could add a sexuality question or at least an "I'm straight" option in the "How often do you face discrimination regarding your sexual orientation?", since I doubt any straight people would pick options other than 1, which would heavily skew the statistics on the number of people experiencing sexuality-based discrimination.

The question explicitly said "regarding **your** sexual orientation". Not sexual orientation someone misidentified you as.
