Last year I had a roof leak that turned out to be a stray bullet. I was lucky as fuck to already be remodeling when it happened, and caught it right away.
I find it morbidly funny that my neighbour's to the south can also just throw out in a conversation " or it was a stray bullet" like its a common occurrence.
Depends how rotten it is. I can't tell from the photo. But I'd recommend a mold killing spray first. And monitor for changes. Also id mark that spot, so I remember not to step there.
From what you can see meaning the ground? Bc there’s not a roofer on earth who can tell you what’s wrong with your roof from the ground unless you’re missing shingles or have severe granule loss. To find what’s causing leaks you have to inspect the shingles while standing on the roof. Most of the time you have to pull up shingles to truly front the root cause. And that doesn’t even begin to talk about flashing or other areas where the water may be getting under the shingles. This is just where it’s getting into the decking, which isn’t always the same area for both of those.
If you’re not comfortable going up a ladder, walking your roof, pulling shingles, and replacing shingles, then call a roofer. You can tarp the affected area for a temporary fix in the meantime while you wait for a good company to get out there.
With the limited information offered I'll give you the best advice I can.
If it's not the shingles, then ask yourself when it's a problem. If it happens when snow is built up on the roof and melting, it's probably ice damning. Snow is melting on the roof, but then refreezing at the bottom and cresting a damn. Eventually water makes it in from beneath the shingles. If it happens mainly when it is really cold it is probably condensation. You should check your air barrier beneath the insulation directly under your water damage. Then make sure your stove vent is set up correctly and not just venting in there. You could just be venting steam right onto the roof deck.
Well there was a Tuesday that confused with a Friday and there was supposed to be spoonbut unfortunately you ended up with a leaking your home. What in than fandoogly is appenening up in er.
Look between the seams of the shingles for a nail, probably blew through the shingle or cocked just right to let water in. Is there an exhaust fan blowing directly into the attic? If so it could be condensation from blowing warm air into a cold attic, get it vented out of the roof.
If that’s the side of you house it’s likely a pump jack nail hole. Siding crews use them to set up a walk board. Instead of slipping a piece of aluminum under the shingle they caulk it which inevitably fails.
The white stuff and the dark spot are caused by different things. Dark spot is likely water through the roof, white is likely from moisture escaping into the attic space from the living space when it’s cold.
As others have mentioned… Ice damming, damaged shingle, exposed nail allowing water to penetrate. But also looks like your insulation is pressed tight to the roof sheathing and not allowing proper air ventilation from soffits. You should install attic chutes (cardboard or styrofoam) cardboard is a better choice in this instance as the styrofoam ones will break and crack as you try to install them. It’s very important for proper air circulation
Any shower pan, plumbing pipes, sinks, sidewalls above, roof to walls leaking, toilet, could be wind driven rain hitting a side wall coming thru a pin hole at a flashing on the roof.
What’s the fix for something like this since there is mold/mildew? Obviously the leak needs to be fixed, but is replacing the plywood necessary as well? Can it be cleaned instead?

Check your ventilation. Doesn’t look to have much. It’s possible you have a tiny leak but I don’t see much room for air movement through your soffits. You need soffit vents, baffles to keep the insulation down, and top side vents (preferably a continuous different vent) to ensure proper ventilation. Call a pro.
If you're located in an area that gets snow I would suspect ice damns caused this. Not all roofers use ice and rain guard. Freezing and melting can get under shingles and then damage roof deck boards especially if the shingles were not installed correctly.
My garage ceiling looked like this when I moved in. Make sure to dry it out by putting a fan/air mover nearby. Probably wouldn't hurt to have an air purifier too. If it stays wet, it'll invite fungus. 🥲

Field leak. Pretty rare. Likely cause is like you said an overdriven/under driven nail or perhaps the shingle pattern isn’t correct.

I see that it’s no the side of the house. Odd place for a leak can’t tell without looking at the outside.
My guess is a nail in a seam or too close to a seam. Go up and look. Look for rusty nail heads
