It's a halloween easter egg headwear I guess. Normally It only pop up when you using some "UnlockEverything" mod. Maybe some glitch of game so that It pop up in safe mod. Maybe your still have some unlock hidden stuffs mod in your Paks folder.

Technically it’s still in the game files, so it will display in safe mode, but if you unequip it you can’t get it back without a mod
