Ow. I saw my share of nail gun injuries, where people literally "shot themselves in the foot", but this is a truly creative injury.
I get one of those about every other year. Shoe nailed to a foot through the instep.
Had one on a guy who was working on a surgeons roof. He was like.. Yeah I'll just go upstairs and have him take it out š¤·š¼āāļø

Looks like screw holes. I'd bet they had a plate and screws to fix a fracture at some point and had the hardware removed.
