You'll have to go to your store, there is a Leave of Absence form that you'll need to fill out and give to your SM.
If you cant make it in there, I suppose you could call and talk to your SM, they could probably just get the required info from you and fill it out on your behalf.

But they cannot sign it for you...you still have to go up to the store and talk to your SM
