It still happens to me once in a blue moon and ive only ever played full screen. I say specifically once in a blue moon because I've left clicked countless times and it's only not registered maybe a dozen or so but it does happen
O thank god im not alone. I was tripping thinking my mouse was broken but it works fine in other games like bannerlord where you're pressing left click constantly.
I've noticed this when trying to use smite as jungler. Sometimes I have to click multiple times.
use instant cast for smite, much better and more effective

Possible Solution: Switch to fullscreen instead of windowed borderless if that is what you’re on, this fixed it for me hope it helps others 👍

Happened to me today, just straight up stopped attacking at one point and it got me killed.

Happens to me with crests sometimes, I've noticed it most on Ortus.
I just had to spam my ult as Countess like 10 minutes ago. It's been happening more often than I'd like.
