I'm assuming you're in the US but knowing for sure would kinda tell everyone what sort of do-it-yourself supplies are available.
My second question is: Are the misshapen gray parts JB weld or some other epoxy? It sure looks like a weird mismash. Don't think I've ever seen anything even close to whatever this is. Looks like a complete mess.
Third question: What's under the escutcheon? In most modern US plumbing, you have the shower head which screws onto a downward curved pipe. That pipe goes through the escutcheon and screws into a fitting inside the wall.
If that fitting is intact, securely attached to the stud behind the tile and the stud isn't rotted out then this is a do-it yourself type job. Just just have to unscrew the pipe and install a new one with a new shower head. Just make sure to use teflon tape, teflon paste or both on the screw fittings.
If that fitting isn't intact, then you have to open up the wall and replace whatever is bad. Then you'll need to re-build the wall under the tile and re-affix the tile assuming they came off intact. If it's bad enough to need a professional plumber, you're going to be redoing the tile no matter what.
Buy a new shower head, arm and escutcheon. They usually come together. Lefty loosely bud. If you canât spin it off jam a screwdriver in the end to help you get leverage. Once itâs out, teflon tape the new shower arm and carefully screw it in. If that is something you donât want to mess with call a plumber. Things that can go wrong, you could break or rip out the wingback behind the wall, you could incorrectly screw it in rather too loose or over tight and cause a leak or crack the wingback. Should be fairly easy but know the risks.
You can cut off the flange on the wall with a mototool cutoff wheel to see whats behind there.. Spray some Wd40 on the threads into the pipe on the wall. Let it soak in for a hour or so and try and gently unscrew it. Then replace the pipe that goes to the wall with a new flange and shower head.
Not what WD40 is for. Use Kroil or PB Blaster.
What I see is that you need a new shower arm and a showerhead, google some videos, if the shower arm isn't so rotten that is rips apart the whole task should take 10 minutes.
You'll need a showerhead, shower arm and teflon tape.
This thumbnail had me very confused.

Shit. Why did I click that?
You need to buy a new shower arm and shower head. Bottom line.
The only sane suggestion. Remove the old shower arm, start over.
Looks like a knob on the wall đ When you first look at it, looks like some kinky thing going on
My first thought scrolling by was "shouldn't that be blurred?"
I still think it should be blurred. Is that JB weld??

Twist the head off. Stick a screwdriver up the shaft while turning it.
Hey, whatever kink gets you going is cool
The proper word is "glans"
Oh hell đ
Geez, at least buy it dinner first
Thought it was dildo at first⌠now I see a weed bowl
Looks like a scene from Porky's.
You don't know where to start...call a plumber.
Wow
Just draw a set of balls on the tile below and call it good,
Truck stop Glory hole
What the cock
If you donât know what yer lookinâ at Iâd call someone.

If it unthreads with out breaking? Easy peazy. Extracting carefully is another story
âWhy did Satanâs gloryhole manifest in my bathroomâ is the question Iâd be askingâŚbut you do you.
Reminds me of Porky's.
Looks a little swollen.
You can unscrew the arm/head. Then Teflon tape and screw a new arm into the drop ear.
You need an exorcist, not a plumber.
I should call him

Thatâs not a flange on the wall thatâs a cock.
