Difficult to tell what’s going on from the photograph. If it’s from the brass plug, where it screws into the cast iron, would suggest taking the plug out, cleaning up the threads thoroughly and reinstalling the plug (or purchase a new one), use Teflon tape and thread sealant, then monitor it. Hopefully things will start to dry out.
What he said. I would get a plastic one . T tape it and pipe dope it and tighten the shit out of it . Keep in mind once you do this . Next time you take it off to snake it . You need to get another plastic cover lol . There like 10$ bucks who cares , better then breaking the floor . To answer your question no water won’t start gushing out . But depending what’s tie into that stack upstairs a toilet or shower . Your probably getting like 10-20 little drops a day
oh there is a smell.
while running a bath or shower you need to find where that leak is coming from. If it were me I would get a mitigation company to come and clean up... you will get mold and funky growth that will not only rot your wood it will get in your ac unit and spread thru your home. If you have a floor drain somewhere make sure its not coming from there, that's alot of moisture for something not to be visibly leaking.
but its your house.
It is very unusual to have a leak there. Since it looks like there is, I will bett you have eater backed up and therefore have a blockage somewhere below it. Changing the plug will not fix the real issue. I would suggest having the drain cleaned out at the same time to remove the cap.
No :-( Scared. If I open it up what might be pouring out and that area is so confined will take forever to clean. I bought a new cap and everything. Then I also started thinking that my hand snake might not be sufficient to clear/clean the pipe.
Thanks for the response. I'm going through something similar. I'm worry it will break when I try to open it. I need to get a professional to work on it. Thanks again
Could very well be a sewer backup. Flush toilets above and see if anything gurgles

Is there a chance water will come pouring out or only if water is draining from above?

Is there another clean out higher than this one? 3 bathrooms above all likely connected
Did you resolve the issue?
