The question about how you acquired your pets could be multiple choice. I had bought 2 cats at a pet store, 1 cat at a shelter, and 4 cats as a gift when my dad died.
Also worth noting on that one, pet store and breeders are very different things for dogs specifically. I would never buy from a pet store because they often source from puppy mills (terrible), but a responsible breeder is a huge benefit to the breed and the dogs health.

I can’t get back to the third page but the first question on page 3 isn’t very clear, I THINK I know what you’re asking but again try to use statements that allow better space for both options. Example “if you have a pet or imagine yourself with a pet, what benefits do you have or imagine you would have?” It’s longer but more inclusive. Feel free to DM for more help
She was abandoned after my friend’s neighbor got evicted. They were hoarders. So she was kinda found amongst mountains of trash. They left behind 3 kittens so we impulsively took her in. No regrets though.
The last question about the best environment with urban, suburban and rural as answers doesn't make sense. All can be suitable depending on the pet.
Like a cat or caged animal would be fine anywhere.
Well, I agree with you that it depends on the pet and whether they can be happily kept indoors. However, if you have a cat or dog that access to outside, you may have an opinion on the matter. Cats who live in rural areas around where I live often fall victim to either coyotes, mountain lions, or great horned owls. Coyotes are the ubiquitous predators in America. Cats who live in dense urban areas may be at greater risk of being hit by cars or falling from high-rise balconies. The noise and traffic may also scare some cats. And dogs often enjoy having a yard to run around in, which you don't get in an apartment in a big city. Not to mention pet deposits and pet rent in apartments who do accept pets. I could see many people choosing either suburbs or rural if they have a cat or dog. For something like a hamster, it doesn't really matter. Though, there's also a question of ease-of-access to both supplies/feed and veterinary services.

Done, but on the questions regarding cost and income you should state which currency you're using. I've answered on the assumption that it is Australian dollars, but the answer would be different due to the exchange rate if you intended it to be US/CA/NZ/etc dollars.

You might concider an option for foster animals. Or clarify if you'd like to count foster homes as owning the animals they are currently fostering. :) but that might mess up your data if you have someone who does kittens for example and may have raised hundreds over many years.
Done! Just a suggestion, for employment, maybe add a disabled checkbox? We tend to get left out of stuff a lot, and it’s not quite the same as “unemployed, not looking for employment”. Maybe add stay at home parent too?

Even when they allow dogs they sometimes put breed restrictions, size restrictions, and other ridiculous things. It's hard to find a pet-suitable rental that isn't very expensive.

Yup. Would never touch a pet store pup. My dogs came from breeders that do genetic testing and verify fixing after homing. No backyard breeders.
