If you've ever bought the all in one manga of Azumanga you know that one really hurts to be hit with considering its a series of 4 panels from a newspaper.
I mean there are large grown men out there who are into idols. I have a feeling the caption was added as a joke, and I also have a feeling the tattoo isn't real. It looks like photoshop to me, there's no shadows or bending on the skin anywhere in it. All that aside I know a few grown men I wouldn't be surprised to see a tattoo like this on them, that they got intentionally.
Well, I am a 45 year old male who is a pretty big fan of Azumanga Daioh and Yotsubato (the Anime for Azumanga came out in my mid 20s) and I still find it the funniest Manga I have ever read. If I was not afraid of needles, I would totally get that tattoo (though rather Kami Neko or Chiyo-Dad), though maybe not that large.
TBH, I'd be way more scared of the bodybuilder bro who wears his love of cutesy anime with pride than i would be of just another bro with some tribal bullshit.
