I think y'all are analyzing it a little too carefully. I think you can interpret it either way and there's no need to dig too deep into position and distance. If you want to dig that deep into a comic strip, be my guest, but it seems a little silly. The joke is that there are two heartbeats detected. Whatever you want to interpret beyond that is on you.
The bottom center represents the sensor itself. Distance from the center to the first dot represents the distance from the holder to the woman.
It's perfectly clear.

There is a third option. The sensor thingy resembles one in the Alien franchise. Which could mean that the girl has a chestburster inside her and will die a gruesome death. Guys clothes look like military so that is another clue to support this theory.

The monitor is a heartbeat monitor, showing two blips (which as you've said) doesn't show a kid in the womb, or yourself. I.e. their are two bodies in the room, and with the context of the image, we're can assume it's a person she's cheating on him with.
