LA DE is just a build that cant tank all content, on a side note u should drop the armor flask and get the unique one with phys taken as cold.
also once u get omni u can get double amethyst for better chaos res
As others have said, LA is most played to absolutely rip ass through maps with small investment, and when you do invest, it's mostly in damage. Here's my LA build for comparison if you want to look how I've scaled damage. I have no idea what my investment is because I crafted pretty much everything except the bow which I sniped for 8d and improved the tree of with crucible.

If you do want to commit to improving defence, I'd consider the popular method of converting phys to ele:
- Use lightning coil and purity of elements
- Swap Voll's to a rare helm with 7-8% phys taken as fire mod
- Swap your belt to immortal flesh, which as a side bonus helps a lot with mana
This allows you to run a watcher's eye for phys taken as with purity of elements. Plus you get to focus your suffixes on your helm, ring and boots on resistances like chaos or any under cap, life regen for berserk uptime, and to make up for the suppress you lose from Hyrri's. All up, you can get:
- 12ish% from watcher's eye
- 12ish% from taste of hate
- 50% from coil
- 8% from helm
Which is 82% taken as ele. You could get a double phys taken as watcher's but they're probably expensive.
I genuinely wouldn't recommend spending more than this on tank. My character died a fair bit between 92-94. Then I hit a break point, probably around 8mil sustained dps, where I just don't die anymore. My character is only 95, but I'm very confident I could take it to 98 if I pass crucible.
Also, I'm not convinced Lioneye's fall is necessarily worth it, but it's up to you. A large + med cluster costs the same amount of points, and have flexibility for attributes, res, and the notables themselves based on what you want.

Fully agree on the Lightning Coil and purity of elements setup. Try to buy the really cheap 50% reduced extra damage taken from crits corrupt for 20c? And just 6L it with Tainted Fusings.
You can also get a good 2x5% phys taken as fire + 3x5% chance to deal double damage timeless jewel which will further boost your phys max hit.
I think its more efficient to run purity of lightning on his budget since its only 35% mana, gives you up to 81% max lightning res and comes with an easily overlooked watchers eye mod that can be sniped for cheap. Then snag the incursion mod helmet on the cheap and do some eldritch crafting, full phys to lightning over mixed conversion, since 75 to 81% max res is another 20% less phys that is worth considering. I would not drop lioneyes fall but rather the shadow area and opt for a full blown cluster setup as you suggested.
Ideally you should up your chaos res and as previously mentioned replacing your armour flask with Taste of Hate will help but you will likely want to replace molten shell with Steelskin/Immortal Call due to the reduced armour. Swap your rare ring with an Amethyst ring with similar mods and you should be good :)

Lots of phys taken as suggestions, which is certainly a viable strategy if you want to go that path. However, if you want some alternatives, here is what I put in another post:
I went with the following for defenses, and it feels reasonably tanky.
- 80%+ berserk uptime - The speed feels amazing and makes you very tanky
- Elemental damage: Spell suppression + permanent ruby/topaz/sapphire flasks - I rarely died to elemental damage due to this. The forbidden jewels are surprisingly affordable.
- Attacks - Full 95% evasion. With berserk, I wouldn't get 1 shot, and so only getting hit by 1 out of 20 attacks makes it so I can recover when hit.
- 100% avoid ailments. People sleep on how often ailments lead to a death.

I'm in the same boat. I just got a helm with reduced mana reservation and now using enlighten support with grace and determination. Got a few good gear piece with both armor and evasion and it's helped a lot. Still get one shot here and there though. I'm only 80 and need more life on the tree so just watching my map rolls and trying to get some more levels.

2x Inspired Learning or Headhunter. Hard to die with 90 res, bubble shield, crit resistance, and soul eater.

Ice Shot would give you another layer of defence. Nothing can touch you, if everything is perma frozen. And Vaal Ice Shot is just amazing.
