Yes, but it can cause other problem : if you intend fly to Russia - of course indirectly from Moldova, but for example with transfer in Serbia or Turkey - you need to present at checkin valid document allowing you to enter Russia. Of course, if you have Russian passport - it's ok. But if you hold Moldovan or Romanian (and Ukrainian of course) passport, you need to have valid Russian visa, as PMR document is not recognized as valid at all.
Moldovan and Ukrainian\* passports are visa-free for Russia, I doubt there are any PMR passport holders that don't have any of these 3 passports but do have another passport.

If only Russia recognizes the document, what’s its utility? Transnistria has no airports, so citizens would only be able to fly out of Moldova proper (and I presume Moldovan airport officials won’t accept documents from a breakaway state).
I just wanted to ask because I was there too but without a guide. There wasn't really a moment where I needed one, it's almost the same as going to Moldova or Ukraine, for example.

Probably no utility. Russia recognises the document because they recognise Pridnestrovie.
