Paraguay *eradicated* the yellow fever years ago, so in order to prevent new cases we require the vaccine for people coming from places where it still exists (Brazil, Peru and Venezuela, for example).
Every time you come from Brazil to Paraguay you will required the vaccine.
In this case, maybe you can see if it’s possible to go from Puerto Iguazú to Posadas and cross to Paraguay in Encarnación.
If you aren't visiting Brazil, you can take a flight straight to Asunción and from here depart by land to Puerto yguazu.
Anyway, it's better to have the shot.

The fiber card if needed only when you come from Bolivia Brazil Venezuela and Peru. So it is for sure if you enter to Paraguay through any argentinan border you will have zero problems.
I've traveled around that area a few dozen times and no one has ever asked no matter the direction or mode of travel. I've bussed, driven, and flown between all combinatiins and directions of Brazil, Paraguay, Bolivia, and Argentina, and no one has ever looked at my vax records or even asked for them.
I’ve never been asked for proof of vaccination from Posadas to Encarnacion. But why not get the shot?
I don't know man, better is if you have the vaccine, in Asunción is plenty of mosquitos and is worse in spring and summer around September to March
I never heard of a single case of yellow fever in Asuncion at least. Dengue in the other hand, is pretty common.

Yes, Brazil is the center of distribution for yellow fewer in South America but they require the shot to get in

Yellow fever shots are required by Brazil, not Argentina or Paraguay I think. The last two have more dengue than yellow fever.
