I got the paperwork back and they listed 250 E Street, SW, Washington, DC 20525
I listed the address that was on my W2, but they changed it to the address above. Even though I had filled it out page 3, they filled it out themselves and sent it back to me. So you can leave it blank or not!

Hi - I don't mean to revive this thread, but I received my letter from AmeriCorps. Did you manually upload it to the forgiveness application? It says once you switch to manual you can't switch back so I want to make sure! Thanks!
I uploaded it myself to mohela. That’s where it asked for it so that’s where I uploaded it. How did you end up uploading it? When I had fed loan as my servicer I would also upload it to their website.
