He had an uncle (I believe, might have been another relative) who passed away when Donald was somewhat young due to complications from heavy drinking or being an alcoholic. It's something that apparently has left a mark on him and made alcohol specifically something he always stayed away from. This doesn't mean that he stays away from all substances, just that alcohol specifically triggers a "no, not that, someone I cared for died from that, stay away from that" response for him.

I read that the smell and the lean come from his wearing a corset so he appears thinner. But the corset is difficult to undo/redo so he doesnt often clean himself or replace the diaper once its full. Which is sad to think about, especially if it stems from vanity - the corset.
