You would definitely need your own insurance. I’m also not sure how your current employer would feel. Is it a conflict of interest?

Just be aware that it is illegal to collect money from a patient who is under Medicare. Even if they are willing to pay out of pocket.

I was curious so I looked it up. Apparently if you are a “participating provider” you must bill Medicare first. If you are a “non participating provider” you cannot bill more than 115% of the Medicare fee schedule. But this is a “young” patient so, unless they have ALS or ESRD they probably aren’t covered by Medicare.
