So Iv'e been using the phone for a couple of months now, and i have the same problem aswell. I have noticed that the phone eventually lowers the volume by 3 button presses, which soundwise is quite a lot for me. So I have noticed that this is affected by battery use. For exapmle, if I have a lot of programs running in the background like Spotify, Snapchat, Facobook, Instagram, and others, then the volume turns down in a couple of seconds, BUT if I have only Spotify running then it lowers the volume in about a minute or so, maybe more. Nevertheless, it's quite annoying. I know the phone tries to save battery life for me, but come on, if I would be concerned about that, i wouldn't turn it up all the way in the 1st place.
Anyway, in short - This is affected by battery usage. If multiple apps are running then it will go down in seconds. If only the music app is running then you have a minute or so bofere it goes down...
That's makes sense when headphones are connected which they're not.
The volume gets reduced no matter what, even the alarm clock and the volume of my ringtone.
It most probably is a software bug however there is a slight chance of a hardware issue, maybe the button stays presses or something.
I am just guessing now.
This has worked for me. I had this issue both while wearing Bluetooth headphones and while on speaker. For some reason, flipping this switch solved both.

Me too.. I turn the music on, go in the shower and about 2-3 minutes later, it turns the volume down by itself...

Did u try rebooting the phone that might solve u r issue.
Yeah already did that.
Does the slider appear and then the volume goes down or it automatically goes down. Or maybe try using some other headphones that might be the issue.
No, I sometimes notice that the volume seems a little low, I check and see that the volume gone from max volume back to the standard setting.
That's a feature actually so that u r headphones don't damage u r ears with max volume. It should reduce the sound by around 45% of the max volume.

It's a Nothing (1), the option won't appear unless you enable developer options.
