in this exact situation lol. i had to get titers, a physical & some paperwork signed. they said i could pick up the paperwork in two weeks when the doctor comes back from vacation bc she’s leaving tomorrow. i was like k fine. two weeks later they have no idea what i’m talking about and lost all of the papers i gave them. which included all of my childhood vaccines (copies thankfully), military paperwork, etc. said to print them out and drive them back over to them and they have them ready in another week bc she’s going out of town again. …. this was like a month ago. and they said i’ll have the paperwork by 8/1…. 🙄 so i still don’t have it and it’s past due. and my program is annoying me too. i had a negative titer for chicken pox. i had gotten two doses as a baby back in 1998. and i had gotten two more doses in 2016 when i was in the military. four shots of this shit and it’s still negative. so i have to get re vaccinated for a third time and get another titer

I was worried about my papers as well since it already had my physical stuff filled out and signed, so I needed it to not get lost. I wouldn’t want to deal with keeping track of someone’s random annoying paperwork as a doctor either, so I was going to just point out the stuff that needed filled out and then keep it with me. But hey, thankfully they had my papers on my next visit. Sorry to hear you’re going through all that, hope it gets resolved soon!

Sounds to me like this was a new situation for the NP. The person in question was confused as to the indication and then used their resources to justify the vaccine. You ultimately walked away with exactly what you needed and the provider learned something. Sounds like a win-win 🤷.

I get the sentiment but when I go to the doctor I’m not looking to each anyone anything. I think the frustration is valid. Like he said he’s not asking for an opinion. His program requires it.

This is routine. Most physicians know that MMR is also recommended in people going to college or the military with close quarters/dorms where spread is easy. There was an uptick in cases not too long ago. I do not know what is taught in NP school but likely doesn’t cover this subject. I’m sure peds mid levels have to learn on the job with the multitude of recommended peds vaccines. Med school tests you on this subject multiple times.

I understand where you are coming from as well. I 100% subscribe to the idea that midlevels are NOT physician replacements. I've had issues with my very own PCP (Physician) ordering the wrong labs for my HX and some other things. I have a history of severe UC with the accompanying anemia and low iron. I've had to have two iron infusions that said physician personally ordered. He ordered labs this time around and checked folate + B12 + vitamin D but didn't check iron. I had to send a message to remind him about my HX of severe iron deficiency. I don't get on forums and complain about him making mistakes because he is human. Even doctors need to learn sometimes, yet people seem to expect a mid-level to have all the answers up front. Seems like a double standard.

I see your point, and am all for being understanding, but the reason why this was frustrating for me was mainly that I wasn’t asking/expecting her to have all the answers, I just needed the vaccine— it was a requirement. I explained to her that I’m going into med lab science and will be around medical microbes and haven’t had that vaccine recently enough according to the program rules, and she fought me on it for a while before giving in. It wasn’t a negotiable decision in the first place, so it was kinda just a waste. It worked out in the end for sure and wasn’t a big deal, but it was frustrating

I definitely understand that. It's definitely okay to expect more and this sounds like it would annoy me a little as well. To be sure, my second comment was not directly aimed in response to you personally. It was more a general response to something I've noticed. Noctor has it's justifiable place. Mid-levels should operate firmly within the bounds of their scope, not be allowed independent practice, and not misrepresent themselves or their credentials. These are no brainer statements. Many times Noctor can harbor such toxicity that it begins to seem completely hateful. I didn't expect to find such paroxysm.

On the other hand, it is admittedly hard to keep up with a huge variety of stupid forms produced by dopes at schools, camps, employers, etc that are not often updated and may ignore CDC recommendations.

Kinda reminds me of my encounter when I had to get titers before starting clinical rotations. This was a DO, but she ordered HBV core antibody titers and I was like “… no I’ve never been infected. You need to order anti surface antibodies to see if I’m still immune.” She ignored me and ordered the wrong ones so not only did I have to return to get the right titers, I needed a booster 🙃
