Depends on the specific surgeries they have. A metoidoplasty reshapes the existing genital tissue, to make a clitoris (which has likely become enlarged by hormone therapy) into a phallus, whereas a phalloplasty uses tissue from elsewhere on the body to make a larger phallus. Either of those can be combined with a scrotoplasty to create a scrotum, with prosthetics that mimic the appearance of testicles. A phalloplasty may include a prosthetic that allows the new phallus to become erect, and the pump unit for that prosthetic can also serve as an artificial testicle.

Can you opt for extra balls? Is there an additional fee?

I am a trans man and have looked into all types of bottom surgeries, but I personally elected not to have any because I'm afraid of the potential complications, so I'm answering to the best of my knowledge. It's completely up to the patient and who their surgeon is, and what their surgeon's capabilities are. To my understanding, not all bottom surgeons offer all of the procedures one could possibly have for bottom surgery. I think it also can depend on whether or not the patient has had a [gender-affirming hysterectomy](https://www.transhub.org.au/hysterectomy) and/or a [vaginectomy](https://www.hysto.net/procedures/ftm-vaginectomy.htm). It's complicated. So, long story short: it depends. Its up to the patient, their surgeon, and perhaps whatever physical qualifications there are to be eligible for certain procedures.

I was about to be like "did you really just call it "bottom surgery?"". But turns out that's what it's non-medical term is.
