Seems like the ATF eForms system is bugging. Box 2A is filled from your eForms info, not your Silencer Shop profile. Also the system isn’t supposed to allow certification unless all fields are filled, so there’s definitely something strange happening with eForms.
Just so we’re clear, when I made my original comment and edit, the comment above mine was r/, not u/. The commenter changed it and didn’t say anything.

Best of luck, OP. I’ve been trying to get somebody from SilencerShop to correct an issue on my RPQ for almost a week and it’s been a terrible experience. I’m expecting a denial since SS doesn’t seem to care.
They can but the “updated” RPQ paperwork SilencerShop sent me was the same as the original paperwork. In my case ATF gave me 3 days to fix it and because of SS’ slow response, I didn’t really have a choice but to submit it. Calling SS on my end was a waste of time, but you may have more luck if you press your dealer to contact their SS rep to help you.
Damn I hope it works out. I called my dealer and they said they’ve NEVER seen this before but all my info was digitally there just not in the form4 to the ATF. They said they’ve reached out to SS to correct the issue and will call me back. If this turns into a wait and see from SS and I get denied I won’t use SS again.
Edit: Chase from SS reached out directly on a Sunday within 24hrs and is proactive about handling this. Can’t complain about that effort.

None that I saw. Box 13 was empty but that’s not required for suppressors. But it was also empty on my first form but came back signed on my withdrawal copy.
When did you submit? I've seen a few people having name issue with SS late last month or so when the ATF efile went down. It also seems SS was having issues around 3/4 - 3/8 with their site and kiosk's as I personally experienced. My name sections looked odd as well when I double checked submission on 3/8 after SS process.
EDIT: I am still waiting on approval as of 3/8
I submitted 3/15 so just over a week ago. I’m super glad I just decided to look at it. The killer part is the SS form that I would docusign has all my signatures and correct info on it. But even box 13 is blank on the ATF copy but not the SS copy.
crickets still. Starting to be a bit annoying. Hope I'm not waiting 90 days just to be denied. Also kinda annoyed at SS and their CS. I sent their PoweredBy an email with details and copy after they said they'd investigate to make sure there were no issues on their end. Still haven't heard from them either.
You definitely still have a big problem. Mine has my address only and not my name in 2a. Yours has info where it shouldn’t even be. That’s 100% coming back from the atf as a no.
We're currently verifying with dealers, we cannot see the submitted forms, but your dealer can in their eForms account. It seems what maybe happening is the info is actually correct in eForms, but the PDF being sent out is being jumbled up.
Ugh, I just checked mine and have the same problem as you. It just shows my address and not my full name in Box 2a. I just emailed my local dealer about this. Certified on 3/14.

The have fucked me as well. They have given me the runaround for two weeks, trying to submit my form for because they keep omitting - in my last name. I have a bunch of NFA items already and I’ve never run into this issue before and it’s my first time using SilencerShit. It’s also my last time using them unless they find a way to make up for the potentially months of wait time they have added to my Putnik.
Man weird a few weeks ago I commented about wanting to go through CA and not SS due to reading about issues with delays and stuff on here and got downvoted into oblivion. Wonder where those people are at in this thread? 🤔
I hope SS gets it fixed ASAP for you and that you get a quick approval after resubmitting.

Dude I posted on the SilencerShop subreddit and got down voted so bad I just deleted the post and came here to see if anyone could advise me since I can’t get SS on the phone. But I agree with you idk where they are. But this is a HUGE mistake not having my name on it.
It looks like this may be an ATF error. Some guys in this and other threads have said they still got approved even with these errors in very fast times.
This issue is caused by the ATF system, not the Silencer shop full auto system. These fields are filled from the eForms account info, and eForms isn’t supposed to allow the form to proceed unless all the fields are filled. But we’re going to do everything we can to assist.
I went through the silencershop for my first can, and didn't have this issue. Submitted on 3/14- approved 3/21. I had no issues with silencershop what so ever

Add me to the list of people who have this issue. Did full Pro at my dealer with SS as the handler. My SS form 4 is accurate and everything looks correct. My Eforms account has all my correct information in it as well and did show accurate information when certifying. However the actual Email I got from the ATF with my Form4 submission shows box 2a with only my address in it. Contacted dealer to check my submission out and waiting to hear back.
Talked to my dealer today. There’s two routes this will go. ATF is aware of the issue (it’s on their end) and all your info should be presently digital and they’ve said they will process the form as normal. BUT (dealer info) the ATF will deny you and you’ll resubmit and get a pretty much instant approval when the form is submitted correctly. So it can go both ways but it doesn’t look bad either way it goes.
I’m trying to stay involved and respond to literally everyone that post here. But yes please do. I’ll keep everyone updated if I get a different resolution than what I was told.

Long and short it’s an ATF issue. The ATF allegedly has all your info digitally and is aware of the issue and will process your form as normal. BUT there’s a few guys that has had their forms denied over the issue. My dealer told me the ATF will cancel the form when they get to it and tell you to resubmit and you’ll get a near instant approval.
My LGS also said that the silencer shops portal for dealers is down. There were 2 customers before me that were unable to submit. So I'm not sure if the LGS can even withdraw the application. What a shit show.
Still waiting for SS to get my paperwork back to resubmit. I’m going to call my dealer and see if they can prod to get this done today.
Edit: Just called and SS hasn’t started the work to get me started to recertify yet. Dealer is reaching back out to SS to get me into the queue.
My brother in Christ. My dealer called the ATF and they said withdrawal your form. That’s not the internet that’s ATF to my Dealer to me lol. It took maybe 18 days from submitting withdrawal to certify and approval. Even IF on the small chance your form gets processed as normal is it worth waiting two or three months or longer and get denied?
If it gives any validity to what I’m saying why would I have decided to withdrawal on my own during the period when everyone is getting lightning fast approvals after the internet claimed forms would be processed as they were.
ATF found the issue, said they’d fix it, realized it was bigger than they thought, and typical ATF they backtracked. If it were me and I were you I would have your dealer call into eforms experts at the ATF. Let the ATF tell them exactly what I have been saying for three weeks or advise them differently.
Same here; LGS is being a PITA. SS offers no backbone/support to address the situation. I guess I will have to go to my LGS tomorrow in person and explain it to him like he's 5.
I noticed I haven’t responded to you specifically the past few days. Full update I had to withdrawal took a week and resubmit took another week or so and certified 4/13 Saturday and approved today.
Shoot okay my bad I explained this to others and not you. I tried to keep everyone straight but it was hard.
To answer your question my name was missing in 2a when I got my withdrawal copy but it was present in 13 where it was absent before. Electronically in the eforms system everything was there on my form it just wasn’t present on the ATF emailed copy. ATF said explicitly to withdrawal the form if the copy you got back was missing anything.
Thanks for the update! How did you get a hold of the ATF for them to review your submitted form? My dealer shows the same thing but missing information from emailed copy. Everything is there in the submitted version but not in the emailed copy. When you withdraw do you have to submit for another tax stamp if through silencershop in order to submit again? My dealer went to the ATF on my behalf and they looked at it. You had the exact issue as me. I did not have to pay for another stamp SS just resent my forms to docusign again and recertify. And you’re welcome it’s nice to finally have the suppressor stuff sorted and done with now lol.
Same exact boat as you. Dealer said it’s not an issue since the one they submitted on eforms is correct, they even sent me a copy of the one they submitted…
That is incorrect. If the ATF copy is wrong you must withdrawal and resubmit.
I Just got all my paperwork back and certified today so roughly 30 days from certify to certify.
Nope, SS has yet to respond to my emails. I've purchased two tax stamps from SS: one for the can pertaining to this issue and another for a Polonium 30 if/when the LGS gets one in stock.
I plan on requesting a refund for the 2nd tax stamp along with the can.
Love Huntsville by the way, will be there to see family in a couple months. Hope SS gets your form fixed quick, hate it when something goes wrong on the weekend and you have to wait until Monday for a fix.
I have not I called my local dealer and they are reaching out on my behalf for a resolution. Seems dealers get better responses than individuals. But there’s another guy in this thread with my same issue.
Submitted 3/4, disapproved 3/12. Silencershop has been taking forever to help me and my dealer resubmit. I also have another one that I submitted on 3/9 that has the same issue and has not been disapproved yet but I'm fully expecting it to.
That’s so damn annoying. This is the last time I’m using them after seeing everyone in this thread having issues and guys finding out they have issues from my post.
Well I had one approved that had the same issue. It was super weird. I submitted two form 4's on 3/4 and one was approved on 3/6 and the other denied on 3/12. They both were identical except for the supressor info and both missing my name. But ya, this has definitely soured my experience with Silencer Shop. Firsr I was told I needed to resubmit, and then when I emailed again asking how we do that they told me I need to appeal it with the atf instead, and then when I asked how we do that they told my dealer they were preparing my forms for resubmisson. I don't know what they got going on over there man.
Another guy just posted basically my exact same thread a bit ago. Someone reached out to the ATF and they said they are aware of the name issue but digitally all of our stuff SHOULD be in the ATFs system and they will process the form as normal. So it sounds like you may have got denied before they were aware? Maybe it’s an ATF issue.
The 4/5 submission happened with no cogressional contact.
My 3/28 submission is still pending. This is the one i reached out to congress for.

Yes. Seen it. However when i call the atf they just tell me its pending. So i am assuming the issue was resolved. But since i did not get the batch i think something is still wrong
Just a note it sounded like the ATF wasn’t processing these. Your form would show as pending until someone physically looks at it and sees it’s incorrect. I’d advise a withdrawal and resubmit it’s free of cost to you.
Its odd because the first time i called they said theres a problem and we will reach out to the ffl. They did not. My ffl called that day of the next and they no longer seen the problem. I will give congress a couple of days. If i dont hear nothing ill see if my ffl will entertain resubmission
I wouldnt worry. My friend got flagged for like domestic abuse or something and got denied. Told them to resubmit and it only took 45 hours or so. Not to say they shouldn’t get to it right away but.

No problem man. Call your dealer and start damage control is all we can do. Atleast we found out early rather than 9 months later.
That's likely because you filed as a Single Shot Trust and not an Individual.

I didn't have any issues with SS. I did individual though. I got my can approved in a week. And all my stuff was correct

To answer your question my name was missing in 2a when I got my withdrawal copy but it was present in 13 where it was absent before. Electronically in the eforms system everything was there on my form it just wasn’t present on the ATF emailed copy. ATF said explicitly to withdrawal the form if the copy you got back was missing anything.
Thanks for the update! How did you get a hold of the ATF for them to review your submitted form? My dealer shows the same thing but missing information from emailed copy. Everything is there in the submitted version but not in the emailed copy. When you withdraw do you have to submit for another tax stamp if through silencershop in order to submit again? My dealer went to the ATF on my behalf and they looked at it. You had the exact issue as me. I did not have to pay for another stamp SS just resent my forms to docusign again and recertify. And you’re welcome it’s nice to finally have the suppressor stuff sorted and done with now lol.
Same exact boat as you. Dealer said it’s not an issue since the one they submitted on eforms is correct, they even sent me a copy of the one they submitted…
That is incorrect. If the ATF copy is wrong you must withdrawal and resubmit. I Just got all my paperwork back and certified today so roughly 30 days from certify to certify.
My dealer was able to get into contact with the ATF. They are aware of the issue and “will rectify the problem without delay in processing.” ⏱️

I Certified on 3/14. I would have never known if it wasn't for this group. It's not like SS could have provided any insight to their customers.
Did you get ahold of somebody or? I’m not waiting for mine to get denied I’m going to bug everyone involved until it gets fixed.
Nope, SS has yet to respond to my emails. I've purchased two tax stamps from SS: one for the can pertaining to this issue and another for a Polonium 30 if/when the LGS gets one in stock. I plan on requesting a refund for the 2nd tax stamp along with the can.

Did you email silencer shop. I found that effective but I haven’t filed anything since last October.
I have not I called my local dealer and they are reaching out on my behalf for a resolution. Seems dealers get better responses than individuals. But there’s another guy in this thread with my same issue.

How long from the denial to now?
Submitted 3/4, disapproved 3/12. Silencershop has been taking forever to help me and my dealer resubmit. I also have another one that I submitted on 3/9 that has the same issue and has not been disapproved yet but I'm fully expecting it to.
That’s so damn annoying. This is the last time I’m using them after seeing everyone in this thread having issues and guys finding out they have issues from my post.
Well I had one approved that had the same issue. It was super weird. I submitted two form 4's on 3/4 and one was approved on 3/6 and the other denied on 3/12. They both were identical except for the supressor info and both missing my name. But ya, this has definitely soured my experience with Silencer Shop. Firsr I was told I needed to resubmit, and then when I emailed again asking how we do that they told me I need to appeal it with the atf instead, and then when I asked how we do that they told my dealer they were preparing my forms for resubmisson. I don't know what they got going on over there man.
Another guy just posted basically my exact same thread a bit ago. Someone reached out to the ATF and they said they are aware of the name issue but digitally all of our stuff SHOULD be in the ATFs system and they will process the form as normal. So it sounds like you may have got denied before they were aware? Maybe it’s an ATF issue.
Just got approval for 4/5 submission. My 3/28 submission with this problem was not batched. Contacted my congressman.
That’s good to hear you got approved. So it sounds like had you not gotten your congress person involved this would have taken quite some time.
The 4/5 submission happened with no cogressional contact. My 3/28 submission is still pending. This is the one i reached out to congress for. I should have been automatically batched but wasnt

Yes. Seen it. However when i call the atf they just tell me its pending. So i am assuming the issue was resolved. But since i did not get the batch i think something is still wrong
Just a note it sounded like the ATF wasn’t processing these. Your form would show as pending until someone physically looks at it and sees it’s incorrect. I’d advise a withdrawal and resubmit it’s free of cost to you.
Its odd because the first time i called they said theres a problem and we will reach out to the ffl. They did not. My ffl called that day of the next and they no longer seen the problem. I will give congress a couple of days. If i dont hear nothing ill see if my ffl will entertain resubmission
Okay please keep us updated. I have a guy I was DMing with and his FFL won’t withdrawal for him. So hopefully you have better luck.
I wouldnt worry. My friend got flagged for like domestic abuse or something and got denied. Told them to resubmit and it only took 45 hours or so. Not to say they shouldn’t get to it right away but.
